Over the past two years, competition on the Russian retail pharmaceutical market has notably intensified. Significant changes have emerged in the structure of the TOP-10 corporations. In particular, Nizhpharm Group, which led the ranking in 2025, moved to second place in the first seven months of this year, ceding leadership to another group of companies — Servier (including Egis). The former retail market leader, the German concern Bayer, lost two positions over the year and now closes the top three.

In the first seven months of 2026, retail sales on the Russian pharmaceutical market increased by 11.3% compared to the same period last year, reaching RUB 1.17 trillion (at retail prices, including VAT). At the same time, physical demand decreased compared to the previous year: Russians purchased 2.72 billion packages of drugs, which is 2.9% less than in January–July 2025. According to the database "Retail Audit of Drug Sales in Russia (total sell out)" from the analytical company RNC Pharma, more than 810 different companies were represented on the retail market, but nearly 28% of sales were provided by players in the top ten.

The loss of patent protection for the key drug Xarelto deprived the German concern Bayer of its long-standing leadership, causing the company to drop to second place by the end of 2025, ceding the lead to the Nizhpharm Group. The latter led the ranking from August 2025 to May 2026, after which Servier took the top spot. Its success is largely due to seasonal factors: demand for the leading phlebotonic Detralex traditionally grows in the summer period. Compared to January–July 2025, ruble revenue from Detralex sales increased by 10%, and the volume in packages by 8%. Additional growth drivers in the manufacturer's portfolio were the brands Triplixam (+26%) and Velaxin (+57%). The dynamics of Nizhpharm over the seven months of this year are practically on par with the leader in monetary terms; however, physical demand in retail still decreased by 3.4% compared to January–July 2025. Among the company's brands, the antihypertensive drug Edarbi leads, with sales growing confidently in both rubles (+24.5%) and packages (+15.5%).

Within the top ten during the reporting period, only Abbott and KRKA retained their positions, holding fourth and sixth place, respectively. The Slovenian company showed higher growth rates (+15.7%). The most striking event in the TOP-10 was the rapid rise of the Russian company Promomed, which climbed 25 positions and secured fifth place. Over the year, retail purchases by Russians of the company's products more than tripled, while physical demand increased by 74.6% in packages. The main contribution to this success came from the drugs Tirzetta and Velgia. Tirzetta topped the national brand ranking: in the first seven months of this year, retail sales of this drug exceeded RUB 20 billion, which is 17 times higher than in the same period of 2025.

Another domestic company — Ozon Pharmacevtika — dropped one position to ninth place. Over the year, sales of its products increased by 4% in rubles, but sales in packages decreased by 8% due to declining demand for key products. In particular, the physical volume of the leading Bisoprolol decreased by 18%, and Atorvastatin by 23%. Binnopharm Group also dropped to tenth place, despite an increase in monetary volume (+9%), with the company's physical dynamics in retail being negative (-3%).
