Retail sales of the first generic apixaban drug are still two orders of magnitude behind the originator

From January 1 to August 23, 2026, 6.6 million packages of apixaban drugs were sold on the Russian retail market for a total of RUB 11.9 billion (at retail prices, including VAT). The growth rate compared to the same period in 2025 is +5.1% in physical terms, while the monetary sales volume increased by 4.4% over the year. Relatively modest growth rates have persisted for the second consecutive year; in 2025, compared to 2024, the dynamics were recorded at +2.3% in packages and +6.7% in ruble terms. Until March 2026, the only product under this INN was Eliquis, whose marketing rights in our country belong to Pfizer. However, in March of this year, the first generic product entered the market — Apixanta from the Israeli company Teva, which is contesting the patent.

According to the "Retail Audit of Drug Sales in Russia (total sell out)" from the analytical company RNC Pharma, the first sales of Apixanta were recorded on March 22, 2026. On that day, a total of 5 packages of the generic product were sold in Kirov Oblast and the Udmurt Republic. A week later (March 29), sales were recorded in 11 federal subjects, but the total volumes were insignificant, at 21 packages. A noticeable jump occurred only a month after the first sale — on April 20, 2026, Russian pharmacies sold 161 packages of the Teva generic; after that, demand showed a steady upward trend. As of the first 23 days of August, average daily sales of the drug are recorded at 582 packages. During this period, the drug could already be purchased in 70 Russian regions, with the highest volumes recorded in Krasnodar Krai, Novosibirsk Oblast, and Ulyanovsk Oblast. The absolute peak of daily demand for Apixanta also occurred in August; on the 21st, pharmacies sold 754 packages of the drug worth RUB 1.1 million (at retail prices, including VAT).

It should be noted that the current sales volumes of Eliquis exceed those of the generic by more than two orders of magnitude. From January 1 to August 23, 2026, the original drug accounted for approximately 99.4% of the category's monetary volume and approximately 99.2% of its physical volume. At the same time, the emergence of the generic has essentially not affected the demand dynamics for the original product — the volumes are incomparable. For comparison, during the first 23 days of August 2026, pharmacies sold an average of more than 28,000 packages of Eliquis per day. No significant switching to the generic is occurring, partly due to the relatively small price difference — the Teva drug costs about RUB 1,450 in pharmacies, only RUB 350 less than the original product. However, major changes regarding apixaban may occur in February 2027, when the patent currently being challenged by the Israeli company is set to expire. The total number of generic products with active marketing authorizations currently amounts to several dozen products. Their mass entry into the market will almost certainly trigger a price war and an outflow of consumers from Eliquis.
