The highest level of antihypertensive drug consumption is recorded in Russia’s “aging” regions

The Russian retail market for drugs used to treat arterial hypertension* shows stable growth, remaining one of the most capacious categories. In the first five months of 2026, total sales in these groups increased by 13.8% in monetary terms compared to the same period last year, reaching RUB 67.4 billion (at retail prices, including VAT). Physical consumption in the current year also showed positive dynamics, increasing by 2.3% to 179.3 million packages sold, confirming a steady expansion of consumer demand. For comparison, in the full year 2025, total retail sales of blood pressure drugs amounted to nearly 421 million packages, with a total value exceeding RUB 146.7 billion.

According to the "Retail Audit of Drug Sales in Russia (total sell out)" from the analytical company RNC Pharma, the leading role in the antihypertensive market structure is retained by drugs affecting the renin-angiotensin system, which account for 53% of the category in physical terms and 69% in value terms. Second place is consistently held by beta-blockers (29% of all package sales), while the remaining groups account for only 18% of physical demand.

In the first five months of 2026, the average consumption level of antihypertensive drugs in Russia was 1.2 thousand packages per 1,000 citizens; in fact, each resident of our country purchased 1.2 packages. The highest per capita demand is recorded in Oryol Oblast, where an average of two packages per person, or 1.95 thousand packages per 1,000 people, were sold in the first five months of this year. Compared to the same period last year, the region showed consumption growth of +0.4%. Second place in our ranking is occupied by Tambov Oblast, where from January to May of this year, an average of 1.89 thousand packages per 1,000 people were sold. The TOP-5 also includes Ryazan, Penza, and Smolensk Oblasts, where 1.62 to 1.74 thousand packages per 1,000 people were sold. It is worth noting that Smolensk Oblast showed the most noticeable increase in per capita consumption (+2.5%). The main reason for these regions' leadership in hypertension drug consumption is the demographic aging of the population. According to Rosstat data, these regions are among the federal subjects of the Russian Federation with the highest proportion of elderly people, for whom high blood pressure is a common diagnosis.

In the capital region, demand for antihypertensive drugs was noticeably lower: in Moscow, the per capita indicator for the reporting period did not exceed 910 packages per 1,000 residents. In Moscow Oblast and Saint Petersburg, the consumption level of these drugs was recorded at higher levels of 1.3 thousand and 1.2 thousand packages per 1,000 people, respectively.

The lowest consumption level in early 2026 was recorded in the North Caucasus Federal District, with the absolute minimum noted in the Chechen Republic, where the indicator was only 64 packages per 1,000 residents in January–May. The list of regions with minimal consumption also includes the Republic of Tyva and Nenets Autonomous Okrug, with results of 432 and 494 packages per 1,000 people, respectively. Low indicators here also correlate with demographic factors, as these regions are recognized as the "youngest" in Russia: the average age of the population in Chechnya and Tyva is about 29 and 30 years, respectively, while the national average is over 40. At the same time, the sharp increase in sales volume in the Chechen Republic (+20.5%) is obviously associated with the completion of the implementation of online cash registers and does not reflect real consumption dynamics.
