Physical consumption of phlebotonics in Russia is declining, while consumers are shifting to more expensive drugs

In Q1–Q2 2026, Russian consumer spending on phlebotonic drugs increased by 8.3% compared to the same period last year, exceeding RUB 20.3 billion (at retail prices, including VAT). This dynamic is driven by the continued rise in the average package price, which increased by 12.3% over the year, surpassing the RUB 782 mark. At the same time, the long-term trend of declining physical consumption persists: in the first half of 2026, the physical sales volume decreased by 3.6%, to about 26 million packages. A similar trend was observed in the previous year, when the market volume in packages declined by 3.3% to 52.1 million packages, while the value volume increased by 12.5% to RUB 37.2 billion.

According to the "Retail Audit of Drug Sales in Russia (total sell out)" from the analytical company RNC Pharma, the phlebotonic market structure is characterized by a significantly larger share of systemic drugs, which account for more than 72% of sales volume in monetary terms, while their physical consumption volume is only 41%. It should be noted that the segment of topical phlebotonics has been in a state of long-term stagnation, showing annual declines in physical consumption since 2023. The decline in demand for systemic dosage forms began later — only last year, when sales in packages decreased by 2.9% compared to 2024 levels. In the first half of this year, the downward trend intensified in both segments: physical sales of systemic phlebotonics decreased by 3.3% compared to the same period in 2025, while the decline in the topical category was -3.8% in packages.

In January–June 2026, the retail market assortment of systemic and topical phlebotonics included 72 brands, with Detralex (Servier) remaining the sales leader. Despite intense competition from numerous analogues based on the active ingredient hesperidin+diosmin, this drug consolidated almost 32% of the group's value volume. In the first half of 2026, consumer spending on its purchase approached RUB 6.5 billion, showing growth in both monetary (+10.4%) and physical terms (+8.1%). Second place is held by the domestic brand Venarus (Binnopharm Group), whose share during the reporting period was 11.4% of value sales. The brand demonstrated high dynamics: revenue increased by 16.6%, while physical consumption grew by 9.2%. The most notable change in the top tier was the shift of Flebodia from Innotek to fourth place with an 8.1% ruble share. Against the backdrop of a 2.9% decline in physical sales, the brand shows modest ruble revenue growth (+3.8%). The vacated third place was taken by the Troxevasin line (Teva), which combines systemic and topical dosage forms. The brand's products accounted for 8.4% of the category's total value volume. Even with a 2.4% decline in physical demand in packages, the brand's financial indicators increased by 9.1%. The highest ruble growth in the top ten brands was recorded for Heparin Ointment, manufactured by seven companies during the reporting period, the key one being Nizhpharm. Total consumer spending on the drug increased by 18%, but physical consumption dynamics were not as high (+1.7% in packages).

It is also worth noting that in recent years, systemic drugs have been registered in Russia more often than topical products. Over three years, about five new products have appeared on the market, almost all of them domestically produced, primarily containing the combination hesperidin+diosmin. Among the new products presented, the highest sales volume during the analyzed period was recorded for the brand Roklis from Artelar. Moreover, the brand showed sales growth of 39%, allowing it to secure 11th place in the overall category ranking.
