The physical sales volume of finished medicinal products in the retail segment of the Russian pharmaceutical market in the first half of 2026 amounted to 2.4 billion packages, which is 3% lower than in the same period last year. At the same time, spending in the category increased by 11%, exceeding RUB 1 trillion (at retail prices, including VAT). This growth was driven exclusively by prescription drugs — over the year, their sales increased by 15.8% in rubles and 0.6% in packages. In contrast, the over-the-counter segment faced a decline in consumer demand, reflected in a 4.4% decrease in ruble sales and a 5.9% decrease in packages. As a result, the share of Rx drugs in the retail structure rose to 60% in monetary terms and 45% in physical terms.
According to the "Retail Audit of Drug Sales in Russia (total sell out)" from the analytical company RNC Pharma, in Q1–Q2 2026, pharmacies sold over 1.1 billion packages of prescription drugs worth RUB 601.2 billion. During the analyzed period, sales of this assortment from 620 corporations were recorded in the retail market. The TOP-10 Rx drug manufacturers saw significant shifts over the year. The top of the ranking was taken by the Russian group of companies Promomed, which demonstrated a threefold increase in revenue compared to the same period last year. Despite a diversified portfolio of more than 130 trademarks, the main driver of this pronounced growth was the new drug Tirzetta (INN tirzepatide). Its pharmacy sales in the first half exceeded RUB 15.9 billion, accounting for 4.3% of total consumer spending in the prescription segment. Second place in the ranking is retained by KRKA (4% ruble share). In the Slovenian manufacturer's retail portfolio, which includes 74 trademarks, the largest share of consumer spending went to the lipid-lowering drug Roxera, whose sales increased by 47.8% in rubles. The top three by sales share is rounded out by the Servier Group (3.9% in rubles). In the prescription segment, the manufacturer is represented by approximately 50 trademarks, most of which showed double-digit revenue growth. In particular, sales of the flagship brand Triplixam increased by 25.5%.
The domestic company Geropharm showed rapid upward dynamics, moving from tenth to fourth place in the ranking thanks to a 93% increase in total retail sales. The key factor in this pronounced growth was the brand Sedjaro (INN tirzepatide), launched in early 2025. Compared to the first half of last year, sales of this drug in monetary terms increased 87-fold. The only manufacturer with negative dynamics in the top ten Rx segment companies was Bayer HealthCare: over the year, sales decreased by 3% in rubles. The key negative factor for the company remains the expiration of the patent protection for its flagship anticoagulant Xarelto. At the same time, the shift of AstraZeneca to fifth place in the ranking is noteworthy. Despite losing one position, the manufacturer showed confident sales growth of 18% in value terms; Forxiga (INN dapagliflozin) remains the company's key product in the retail market. Despite the start of sales of generic dapagliflozin drugs in early 2026, Forxiga still shows 34% growth in value terms compared to Q1–Q2 2025.
